Bethel, AK: The Last Frontier



Bethel, Alaska, a community of nearly 7,000, is located on Kuskokwim River near the southwest coast of Alaska. Many refer to this and several other areas of Alaska as the Bush because they are only accessible by plane, boat, ATV or snow machine. Comprised of 62% Alaskan Natives, Bethel is an ever-constant growing community.


My experiences so far have been more than I could have imagined. On my two village visits to Newtok (Population: 321) and Chuathbaluk (Population: 120), the Public Health Nurse and I performed tuberculosis skin tests to all of the school children. We also provided the village community with access to immunizations, STD examinations, well-child checks, and education including suicide and STD prevention. In addition, with flu season nearing, I educated the school population on proper hand hygiene and flu prevention. In educating this population, I tailor my teaching to their availability of resources; running water and soap are hard to come by in many areas of this region. When all of your supplies are either shipped in by plane or barge, prices of resources are much more expensive than we would find at home.


While working with this population the greatest challenge non-natives face, especially in delivering health care, is trust. One nurse tells her employees that if there is one thing you can accomplish in your first two years as a public health nurse it is developing trust. Building trust with the native population is critical in your success as a health care provider.


Aside from providing care, my favorite experiences so far have been salmon fishing on the Kuskokwim River and traveling into the Kuskokwim Mountains in Chuathbaluk. I have been told that I have only scratched the surface of what Alaska has to offer, and I am so excited to see what else this beautiful country has in store for me.

  1. Nick, what a wonderful post and photo. What you write is so true. In the villages of Huslia (population 300) and Anaktuvuk Pass (population 250) had many of the same difficulties with limited resources and expensive costs for transport supplies.

    Trust is indeed a huge issue. Many of the villages' health care providers, teachers, and other specialists simply rotate through. It seems like some outsiders are drawn towards work in the villages for the high wages native corporations offer, but the leave once they've made enough money. They never stay long enough to learn the native culture, become part of the community and foster relationships with people of all ages.

    I think it is especially concerning that children in villages have a difficult time in school, and it is possible they might do better if adults in their own village became school teachers. What was your experience when visiting the schools?

  2. I would agree with both of you that often times trust is the deal broker when others from the outside come in as care providers. Building trust and relationships is so essential to partnering with our patients. We have to have genuine concern in interest in the people that we serve and work along with. If the characters/providers are ever changing it will certainly be difficult for them to develop trust. How will this make a difference for you in the care that you deliver in the future not only in Alaska but anywhere?

  3. The schools that I visited did have many long time community members as well as villagers that were teachers/school personnel. However, in Bethel, from what I have heard, that happens a lot. Children, and parents for that matter, wont’ spend the time listening and getting to know you, if you don’t take your time to get to know them. I hear you loud and clear Travis.

    I think that going about your work open minded is very important. Probably the best advice that I have received here was when you first work with someone, forget about the business. Talk with them, get to know them, and make them comfortable. Then you will truly see them in their element and they will work with you. When you enter a room and start rolling off questions, you lose; and for that matter it is rude and I feel disrespectful. You can get anyone to cooperate with you if you simply work with them. The person that gave this advice to me is the nurse at the Bethel County Corrections Facility; makes sense. I feel this is advice that I will take with me no matter where I go.

    In the little time I have been here, I have learned so much. Trust is not an easy thing to give away, but it is the easiest thing to break. This is a thought that I will not only live by in my career, but in my life.
